Works under the supervision of the Care Coordination Director and is responsible for working in collaboration with the healthcare team to coordinate the care and service to patients across the continuum of care. Develops daily team staffing assignments and supports team members with complex patient needs, to include giving guidance, taking the case on directly and/or taking unit assignments. Leads the daily "discharge ready" review. Provides support as needed to allow team members to attend interdisciplinary care rounds, etc. Provides daily oversight of regulatory compliance, such as delivery of MIMs, etc. Mange's the care management portion of the long stay report, assuring accurate and up to date information and facilitating progress towards medical and discharge planning goals for these patients. Monitors referral reports, investigates cases and trends causing leakage and reports out on a weekly basis regarding RRMC referral results. Serves as a role model to the Care Management team by providing a comprehensive range of service or clinical care associated with and in accordance with standards of excellence established by Riverside Health System and facility goals and strategic plans.

Educational Requirements: RN Diploma / Associates, Bachelor's in nursing preferred

Certification/Licensure Requirements: Licensed RN if applicable with Education.

Experience Requirements: Three or more year's inpatient care coordination, discharge planning or case management experience required. Related experience or basic understanding of case management, and health care delivery. Team lead/supervisory or management experience preferred.
