Job Details

Coordination of supports used by people with disabilities. Work will be directly with people with disabilities and often their families. Work will also occur with service providers. Job tasks include assessing the needs of people with disabilities, developing documentation needed for Medicaid services, local travel to individual’s homes, establishing services and advocating for needed services. The job includes a significant amount of documentation in electronic records and managing multiple tasks and contacts with others.

Requirements

Disability advocacy experience required. A person who possesses the appropriate experience such as a person with a disability or possibly a family member of a person with a disability would be an ideal candidate. Must work from a foundation based on the ethics, advocacy and integrity reflective of consumer control and independence of people with disabilities.


Must have the following experience: assessing the needs of people with a disability; researching and coordinating services; and documenting progress and problems with services. 

Must have knowledge of the following: community services; accommodations and modifications needed by people with a disability; Medicaid Waivers; and rights of people with a disability.


Must have the following abilities: communicate effectively verbally, by sign language or cued speech; communicate effectively in writing or by using assistive technology; develop working relationships with persons from diverse cultural backgrounds; remain objective; multitask; work independently under general supervision; participate as a team member in staff activities; demonstrate a positive regard for people with a disability; and have own transportation for local travel (mileage reimbursed.)


Must have the proven ability to work closely with co-workers and collaborate with other organizations.  Experience working independently and as a team member required.


Full time position, 40 hours a week, typically Monday-Friday


Undergraduate degree in a human services field required.


Annual salary $33,264
