Blue Ridge PACE is currently seeking a full-time Home Care Manager, RN.



 



PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.

JOB SUMMARY: Works under the supervision of the PACE Senior Nurse Manager, oversees the day to day operations of the PACE In-Home Services program. Responsible for all in-home services staff, including RNs, LPNs, Schedulers and Office Assistants. Assists leadership in the policy formation, program development and implementation and annual budgeting. Provides supervisory visits to home care staff (RN, LPN, UCP) in the field to evaluate skills, customer service and general performance. Assists with initial, annual and change in condition assessments as needed in the participant's home environment. Coordinates plan of care with appropriate community resources and the IDT. Acts as the liaison between any contract agencies and PACE, including facility settings such as assisted living and nursing facilities. When appropriate, provides direct care and education to participants and families in the home setting and documents accordingly in the medical record. Communicates changes in participant condition and progress to the IDT and Home Care team members. In the absence of the Sr. Nurse Manager may function in that role as requested by the Site Director. Supports and lives out the Riverside Care Difference.



 



EDUCATION:
Bachelor's Degree in Nursing - If hired, employee agrees to obtain his or her Bachelor of Science in Nursing (BSN) degree no later than 5 years (60 months) from date of hire.



LICENSURE:



Licensed RN or board eligible by the State of Virginia or compact state.



Must have or obtain BLS per departmental requirements.



Current valid driver's license and acceptable driving record required upon hire and to be maintained throughout employment.



Must have reliable transportation to make visits to patient's homes as needed.



CPR Certification and First Aid training required and must be maintained current throughout employment.



EXPERIENCE:



Must have minimum of 1 year working experience with the frail and elderly population.



5 years of RN nursing experience in healthcare, preferably in a geriatric or community setting (home, residential or acute) required.



15+ years of LPN experience and/or healthcare management experience will be considered in lieu of 5 years of RN nursing experience.



2 years of managerial experience in healthcare, preferably in a geriatric or community setting (home, residential or acute) required.



2+ years home care nursing experience highly preferred.



ADDITIONAL: Must demonstrate ability to communicate, perform and train the required skills necessary for providing routine nursing care both in a clinic or home setting. Must demonstrate competency and ability to use computer and working knowledge of computer based software as applicable. Must have excellent communication skills both verbal and written necessary to positively engage and interact with staff, physicians, participants and their care givers and facility visitors. Must be professional in appearance and manner, recognize own limits. Must demonstrate strong leadership, interpersonal and problem solving skills with the ability to foster a strong interdisciplinary teamwork environment. Responds and provides direction and guidance in a positive manner. Seek and provide help and guidance as appropriate. Serves as a role model for all staff members to include students, interns, and volunteers. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. Must be able to sit/walk/stand for extended periods of time. Must be able to quickly respond in an emergency situation. Must have the ability to move in and out of the facility unassisted; walk, stand or sit for extended periods of time. Must be able to travel on as needed basis. Ability to analyze data and arrive at logical conclusions for operating and budgeting purposes. Comprehensive knowledge of general nursing theory and practice, to include basic disciplines related to nursing (such as biological, physical, social and medical sciences) with thorough knowledge of the underlying principles of care management, treatment and nursing procedures. Must maintain strong working knowledge of organization, policies and procedures of the facility. Requires higher level of comprehension, observation abilities, demonstrate memory for details, and exhibit independent decision making abilities. To also include knowledge of PACE regulatory policies, procedures and accreditation standards required. Possible exposure to body fluids, needle puncture wounds and communicable diseases. Occasional exposure to toxic chemicals used for disinfection, hot water and specialty equipment that requires knowledge and training. Occasional exposure to chemotherapy agents.



HOURS: To work primarily day shift 8:00am to 5:00pm Monday through Friday. Must be able to work evening shift, weekend shift and on-call as needed based on participant's care needs.
