Riverside PACE, located in Newport News, VA, is seeking a Full Time Home Care Scheduler to join their team.  

PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, Lifeline emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services. Works under the general supervision of the Home Care Manager.

JOB SUMMARY: Works under the general supervision of the PACE Home Care Manager and the direct supervision of the Home Care LPN Coordinator. Responsible for scheduling of Aide services and visits to PACE participants in their place of residence and the PACE Center as needed. Maintains schedule and log to include number of UCP hours and days of provided services, completes timekeeping for UCP staff. Coordinates UCP services provided by contracted agency staff. Faxes referral and plan of care to contracted agencies, collects and verifies, and maintains documentation from contracted agency staff for medical records. Responsible for payment authorization of care provided by contracted agency. Completes monthly reports on service utilization. Assists with orienting UCPs and contracted agency aides to new participants by reviewing plan of care. Assists with follow up regarding concerns and issues relating to services and updates management as appropriate. Communicates changes in participants plan of care from participants and participates in case conferences as requested. Rotates call for Home Care needs after regular business hours.

EDUCATION: Minimum of High School Diploma or GED required. Additional education or certification in an administrative or medical field preferred.

LICENSURE: Certified Nursing Aide highly preferred. Current, valid driver's license with satisfactory driving record required. Must maintain satisfactory driving record throughout employment. Must have reliable transportation to travel to patient homes.

EXPERIENCE: Minimum of 1 year experience working with the frail and elderly population required. Two years experience with scheduling, staffing and/or home care/private duty/personal care experience preferred.

HOURS: Must be able to work primarily 8:00am-4:30pm, additional hours during the evenings, nights and weekends to Home Care after hours call.  May be assigned to work home care patient assignments to meet patient needs.
