Under the direction of the Home Health or Hospice Director, the Home Health and Hospice Team Leader, is responsible for assessing, planning, implementing, and evaluating an organized program for clinical staff education, directed toward the achievement of professional excellence.  


Serves as the primary contact person for clinical staff training and education.  Establishes professional education resources and instructional leadership for outreach programs in the community.  Lead and supervise the introductory process for clinical interdisciplinary teams (home health and hospice).  

Promote communication and collaboration in developing, monitoring, evaluating and revising home health and hospice patient care plans.  
Audit interdisciplinary documentation to assure accuracy and timeliness.

DUTIES AND RESPONSIBILITIES:

  • Supervise the implementation of the patient’s plan of care by receiving referrals, assigning cases, auditing, organizing, overseeing, integrating, and  modifying the resources necessary to reach desired outcomes. 

  • Conferences with Home Health and/or Hospice Director regarding any questions about an individual’s eligibility for services.  Evaluate the effectiveness of the care plan on an ongoing basis.

  • Provide ongoing clinical assessment and provide care to clients and families.  Assure an accurate evaluation of client needs and develop and  implement a comprehensive interdisciplinary care plan that addresses those needs. 

  • Follows current professional standards of practice and agency approved protocols.

  • Provides educational programs to contracted nursing homes, assisted living facilities and other residential setting as requested

  • Audit and maintain current and accurate client records through use of laptop computers and paper/chart documentation in accordance with agency  policies, federal, state, and local guidelines.

  • Assist the interdisciplinary groups in establishing appropriate goals, priorities, and plans of care.  Complete clinical record reviews quarterly. 

  • Participate in data collection, analysis and performance improvement projects.

  • Leads the process and tracks progress for orientation of new clinical team members.

  • Has overall responsibility for the clinical staff development, orientation and continuing education programs.

  • Provides direction and support for initial orientation of electronic medical record documentation.

  • Oversees the compilation and revision, as needed, of the nursing orientation manual.

  • In the absence of the Home Health Director and/or the Hospice Director, facilitates interdisciplinary team meetings with facilities and/or clinical  personnel to promote coordination of care and discussion on interdisciplinary group involvement.

  • Is available at all times during operating hours to assist clinicians as appropriate.  Act as resource to clinicians in the field.

  • Provide staff development including orientation, in-service education, continuing education and evaluation of staff.

  • Monitor the performance and skills of interdisciplinary teams.  Responsible for ensuring competency check-offs of all clinical staff are current and complete.

  • Collaborate with the Home Health Director and/or the Hospice Director for employee evaluations and disciplinary actions.

  • Ensure organization compliance with legal, regulatory and accreditation requirements.

  • Provide professional nursing service as needed to Home Health and/or Hospice.

  • Schedules and participates in on call rotation for both Home Health and Hospice.

  • Insures clinical personnel stay current with clinical information and practices

In the absence of the Home Health Director and/or the Hospice Director, serves as a qualified alternate.  Must be available at all times during the absence of either director.

Requirements

QUALIFICATIONS (Education, Training, Skills and Work Experience):

  • Five years of experience as a Registered Nurse in health care, preferably in home health or hospice.

  • Supervisory experience in health care preferred.

  • Knowledgeable of applicable local, state, and federal regulations.

  • Must have an unencumbered Virginia license as a Registered Nurse.

  • Must maintain current CPR certification (basic life support).

  • Strong computer skills, including Excel, Word, Power Point.

  • Excellent verbal and written communication skills.

  • Demonstrated ability to meet deadlines.

  • Expected to be able to perform and deliver results working under limited supervision.

  • Expected to be able to multi-task, successfully handling various projects and schedules simultaneously.

  • Must have experience with all types of office equipment:  copier, fax, computers, multi-line phone system.

  • Valid state driver’s license

  • Proof of motor vehicle insurance

QUALIFICATIONS

  • Able to speak and understand verbal instructions in English

  • Organized and detail oriented

  • Strong communication and interpersonal skills

  • Current VA valid driver’s license required

  • Willing and able to work a flexible schedule

  • Willing to drive to private homes in greater Virginia Beach area to perform the requirements associated with this position

  • Five Years experience a a RN in Health Care, preferably in Home Health and Hospice

  • An Unencumbered Virginia License as RN

  • Supervisory experience in health care preferred.
