Our Hospice Pastoral Care Counselor is expected to assess and address the spiritual/ pastoral, bereavement and additional individual and/or group counseling needs. Develop and implement individualized spiritual and bereavement plans of care in collaboration with local clergy and the interdisciplinary team. Conduct religious ceremonies, bereavement support groups, and educational programs as appropriate. 

Provides training for local clergy in bereavement support and end of life spiritual care. 

Essential Functions of the Position

- Available to the patient/family, hospice team and volunteers, to assess & address the spiritual/pastoral, bereavement needs. 
- Available to address individual and/or group counseling needs

- Provide spiritual & pastoral counseling services in accordance the plan of care.

- Complete a spiritual / pastoral assessment as indicated and appropriate as soon as possible after admission.

- Provide spiritual / pastoral support in accordance with on-going wishes and needs- of the patient/ family.

- Provides spiritual support as needed and defined by the patient/family.

- Conduct religious services of prayer, worship and rituals for patients/ families as appropriate.

- Offer patients/families of different philosophies and religious beliefs opportunities to discuss and share their thoughts, feelings, beliefs, and values.

- Document counseling provided in the patient's clinical record

- Work with other professionals in resolving spiritual / pastoral / ethical issues.

- Work in a team approach with qualified professionals, as determined by the hospice, to evaluate patient / family response to care.

- Attend IDG conference when plan of care is reviewed.

- Upon request, meet with individual staff regarding personal spiritual/pastoral issues that may affect their ability to function effectively.

- Develop the bereavement plans of care and provide bereavement counseling services in accordance with the plan of care.

- Be available to hospice staff and contracted facility staff for bereavement support.

- Make visits/ phone calls to the patient/ family prior to death and is available to family during the bereavement period.

- Assess patient/family response to and satisfaction with care

- Participate in development of approaches to meet staff counseling needs.

- Develop and maintain contact with key, targeted referral sources toward the generation of new hospice referrals. 
- Develop new referral sources. Provide referral sources with current information on hospice services.

 

- Develop, coordinate and facilitate referrals and intake. 
- Conduct, as needed, a pre-admission visit to interested patients and families to explain our Hospice program.

  • Provide spiritual / pastoral counseling services in accordance with the plan of care.
  • Complete a spiritual / pastoral assessment as indicated and appropriate as soon as possible after admission.
  • Provide spiritual / pastoral support in accordance with on-going wishes and needs of the patient/ family.
  • Develop an individualized spiritual/pastoral plan of care which demonstrates an effort to work in close collaboration with local clergy whenever desired by the patient; provides spiritual support as needed and defined by the patient/family

  • Work in a team approach with the IDG and other qualified professionals, as determined by the hospice, to evaluate patient / family response to care

QUALIFICATIONS

 

  • Ordained, commissioned or credentialed according to the practices of an organized religious group, preferably Episcopal or Presbyterian.
  • Basic computer skills
  • Must possess good verbal and written communication skills
  • Must be able to meet deadlines, work under limited supervision
  • Must be able to climb at least 2 flight of stairs and be able to stand and/or walk for a period of at least 4 hours
  • Valid state driver’s license
  • Motor Vehicle Driving Record satisfactory to Westminster-Canterbury and its insurance providers
  • Proof of motor vehicle insurance
