Job Summary:

 

A.      Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols:

a.       Directs development, implementation, and maintenance of clinical pharmacy services.

b.      Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.

c.       Coordinates the selection and safe use of alternative medications in response to medication shortages.

d.      Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.

e.      Supports the pharmacy's pharmacoeconomic efforts; reviews the use of targeted medications for compliance with current pharmacoeconomic programs.

f.        Provides clinical consultation and clarification to practitioners; recommends evidence-based medication therapy regimens and monitoring plans; suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.

g.       Monitors drug therapy regimens for allergies, contraindications, drug-drug interactions, drug-food interactions, drug-laboratory test interactions, adverse drug reactions, appropriateness of drug and dose, and therapeutic duplications.

B.       Provides education and information on pharmaceutical care and medication therapy.

a.       Provides accurate, adequate, and timely drug information to the professional staff.

b.      Publishes a pharmacy newsletter for physicians and other health professionals.

c.       Provides training and education to pharmacists regarding pharmaceutical care and pharmacoeconomics.

d.      Supervises and instructs pharmacists and pharmacy students.

e.      Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports, and pharmacy interventions summary to the P&T committee.

f.        Participation in collection and analysis of microbiology data and creation of an antibiogram annually.

g.       Reviews and evaluates the medical literature for studies supporting the pharmaceutical care and pharmacoeconomic programs.

h.      Participates in community educational programs (e.g., diabetes classes, health fairs).

i.         Contributes to the advancement of the profession through presentations at local, state, and national meetings

C.      Contributes to the quality and effective operation of the department.

a.       Reviews the performance and documentation of clinical activities of all pharmacists including documentation of the financial impact of clinical activities.

b.      Coordinates policies and procedures for clinical activities (e.g., therapeutic interchange).

c.       Prepares reports on pharmaceutical care and pharmacoeconomic activities including financial impact, as directed.

d.      Participates in the quality improvement activities of the department; collects and analyzes data and maintains documentation of all clinical activities.

e.      Establishes productive, collaborative relationships with staff members and within the community.

D.      Maintains competence required for current job title/position.

a.       Maintains current pharmacist licensure.

b.      Attends management meetings.

c.       Sustains professional competence and promotes development through participation in in-services, workshops, seminars, and professional organizations.

d.      Completes all competence/skills assessment requirements.


Education: B.S. or Pharm D Pharmacy Required. Current and valid Pharmacist licensure with eligibility in Virginia is required.


Experience: 3 years of experience as a pharmacist is required.
