Position Summary 

The LPN is responsible for technical skills encompassing patient care and related tasks under the direction of a Doctor, Registered Professional Nurse, Nurse Practitioner or Physician Assistant. The LPN works closely with patients and medical staff to ensure exceptional Patient Care.


Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  • Delivering direct patient care to patients as assigned, using established procedures
  • Documenting history; recording and reporting pertinent observations and patient reactions to staff, as appropriate; documenting lab procedures and ensuring follow up on results
  • Using screening procedures, including medical history review, and observing and documenting patient vital signs as required
  • Maintaining quality results by running standards and controls
  • Verifying equipment function through routine equipment maintenance and advanced trouble shooting; calibrating equipment utilizing approved testing procedures; monitoring quality control measures and protocols
  • Training other technicians or other workers
  • Assisting with or instituting emergency measures for sudden adverse developments during treatment
  • Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient as evidenced by documentation and observation of positive patient care outcomes.
  • Perform delegated independent nursing functions using established procedures, policies, guidelines, and standards as observed by the registered nurse
  • Responsible for maintaining, monitoring and reviewing Coumadin Patient care to include outcomes
  • Responsible for managing Nurse Clinic schedule
  • Responsible for maintaining and reviewing Home Health orders
  • Contribute to the development of the patient plan of care based on the identified patient problems/needs.
  • Carry out physician orders accurately, directly and indirectly
  • Assist in the evaluation of the effectiveness of the nursing care administered relative to the patient identified problem/needs.
  • Document pertinent observation and intervention on appropriate record
  • Perform selected tasks related to patient care and the patient's environment in a safe manner
  • Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomes
  • Perform limited I.V. Therapy procedures on those patients receiving peripherally inserted Intravenous fluids, as delegated by and observed by the RN
  • Evaluate the infusion site and the extremity for possible infiltration
  • Verify the intravenous according to the prescribed flow rate
  • Perform limited I.V. Therapy procedures upon proof of successful completion of Ohio Board of Nursing approved Intravenous Therapy course, as observed by RN
  • Maintain and coordinate communication among patient/significant other, staff, and physician as evidenced within medical record, client, and health care team feedback
  • Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care
  • Effectively communicate patients changing needs and plan of care among staff members
  • Report patient needs/problems and observations regarding patient’s condition to the registered nurse and physician. 
  • Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback
  • Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Follow proper reporting procedures for actual or potential accidents and/or incidents so follow-up and/or prevention can occur
  • Performing other duties as assigned



Minimum Qualifications (Knowledge, Skills, and Abilities)

Graduate of an accredited school of Nursing

Valid registration as an LPN in the State of Virginia

The noise level in the work environment is usually moderate. The employee may be exposed to some hazards such as radiation, bodily fluids that may contain disease, and fumes from laboratory chemicals.
