This is a PRN "as needed" position. 

The LPN, Nurse Supervisor oversees nursing personnel in Enhanced Services/Assisting Living on an assigned shift and is responsible for ensuring a high standard of care for the residents. Job duties are accomplished under the direction of the Clinical Coordinator and according to accepted standards of practice. The ideal candidate is an LPN licensed in the state of Virginia with some supervisory experience and current CPR Certification.

  • Assists with the planning and overall nursing care of the residents living in Enhanced Services and our memory care community assuming care responsibilities when necessary on assigned shift
  • Provides supervision of Medication Aides in Enhanced Services; assigns work, reviews/revises schedules and assignments to ensure residents’ needs are met
  • Ensures regulatory compliance with the Department of Social Services
  • Provides input to Coordinator in assessing residents for appropriate level of care prior to admission/discharge to The Hoy Center or hospital and monitors residents closely with a change in condition or acute illness
  • Provides appropriate input into service plans to incorporate residents’ needs and preferences, family’s desire for care and interdisciplinary input and provides necessary input for care planning purposes
  • Directs staff as they perform resident care, dispense medications and treatments, and documents according to Enhanced Services policies and procedures and provides technical guidance and direction to Medication Aides
  • Responds to emergency situations
  • Notifies the Enhanced Services Clinical Coordinator/Administrator when incidents occur and/or there is a change in a resident’s condition
  • Supports Hoy Center team in responding to emergencies for independent residents
  • Monitors completion and accuracy of daily MAR/TX sheets, physician orders, labs, and room checks
  • Supports Coordinator in orienting staff to new positions and providing education to maintain and improve competency
  • Conducts performance appraisals and personnel actions for staff assigned to Enhanced Services according to facility policy. Provides feedback and coaching to staff regarding performance
  • Makes recommendations regarding hiring, discipline, transferring, and termination of personnel
  • Assists with periodically updating written policies and procedures that govern the day-to-day functions of Enhanced Services


  • Licensed LPN in the State of VA
  • Current CPR Certification
  • Strong computer skills, including Microsoft Office (Word, Excel, Outlook) and the internet
  • Completed training in Uniform Assessment Instrument and Individual Service Plans
  • Prior supervisory experience preferred
  • Familiarity with DSS regulations preferred
  • Excellent communication skills and interpersonal skills
  • Experience working with an aging population
  • Maintains licensure requirements and seeks new knowledge in order to improve skills
  • Physically able to help with resident evacuation in the event of a fire or other emergency
  • Must be able to lift 25 pounds, work on your feet throughout shift, and bend, kneel, stoop, and reach
  • Able to speak and understand verbal instructions in English
