To work primarily Home Care patient assignments/cases through the PACE-MacTavish facility.
PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.
Works under the supervision of the PACE Home Care Services Manager providing personal care, comfort measures and environmental services to the PACE Participants (including those needing special precautions) in their homes. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age and needs of the patient served and as described according to the departmental policy and procedure. Promotes and maintains safe work environment for PACE staff and participants by incorporating national patient safety initiatives. The Personal Care Aide may be required to work weekends and holidays as needed. Will perform other duties as assigned.
Education/ Certification: Diploma or GED preferred. Completion from an approved personal care aide training program
Licensure: Proof of current, valid driver's license and acceptable driving record or valid ID with reliable transportation.
Additional Requirements: Prior experience in health care preferred. Must have minimum one year working experience with the elderly population.
Hours: 3pm - 8pm Monday - Friday, and every other weekend
