PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, Lifeline emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.
We have an exciting opportunity for a Scheduler. Works under the general supervision of the Home Care Manager. Responsible for scheduling of aide services to PACE participants in their place of residence and at the PACE Center. Maintains schedule and log to include number of aide hours and days of provided services, also assists with timekeeping for Aide staff. Completes monthly reports on service utilization/number of hours provided. Collects and verifies contract aide notes and maintains documentation. Responsible for payment authorization of agency provided care, faxes referral and plan of care to contract agencies and coordinates aide services, assists with orienting PACE aides to new participants and reviewing plan of care; responsible for assisting in follow up regarding concerns and issues relating to aides and keeping home care manager informed of same, communicates requests for changes in the plan of care from participants to home care manager; participates in case conferences as required.
EDUCATION: Minimum of High School Diploma or GED required. Additional education or certification in an administrative or medical field preferred.
LICENSURE: None required; certified nursing aide preferred. Current, valid driver's license with satisfactory driving record required. Must maintain satisfactory driving record throughout employment. Must have reliable transportation to travel to patient homes.
EXPERIENCE: Minimum of 1 year experience working with the frail and elderly population required. Two years experience with scheduling, staffing and/or home care/private duty/personal care experience preferred. Basic computer skills required. Must have the ability to hear and speak clearly in order to communicate effectively to staff and participants.
HOURS: Must be able to work 8:00am-4:30pm and rotates call after hours
