Blue Ridge PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.

PACE located in Charlottesville, VA is seeking a full time Senior Nurse Manager. The Senior Nurse Manager works under the general supervision of the Site Director. Responsibilities include daily operational and management oversight of the PACE Center, direct operational oversight of Home Care and Clinic Services and all care provided by nursing staff. Ensures the completion of the initial, six month and annual nursing assessments of participants by registered nurses as well as the development of the nursing Plan of Care (POC) for each participant. The Senior Nurse Manager is responsible to ensure that continued monitoring of the health needs of PACE Participants is being performed and met by all nursing staff in the home, center and community. Serves as a member of the PACE Interdisciplinary Team (IDT) and regularly attends scheduled meetings. As the senior clinical leader for the Center, in cooperation with the Physician is responsible for clinical and utilization management oversight for all participants. Responsible for oversight of referrals to outside medical appointments for PACE participants are coordinated and services have been authorized. Assumes the Site Director's responsibilities in his/her absence to ensure continuity of operations, to include facilitation of the IDT. Supports and lives out the Riverside Care Difference.


EDUCATION:
Bachelor's Degree in Nursing - If hired, employee agrees to obtain his or her Bachelor of Science in Nursing (BSN) degree no later than 5 years (60 months) from date of hire.



LICENSURE:


Licensed RN or board eligible by the State of Virginia or compact state.



Must have or obtain BLS per departmental requirements.



Current valid driver's license and acceptable driving record required upon hire and to be maintained throughout employment.



Must have reliable transportation to make visits to patient's homes as needed.



CPR Certification required and must be maintained current throughout employment.




EXPERIENCE:


Must have minimum of 1 year working experience with the frail and elderly population.



5 years of RN nursing experience in healthcare, preferably in an acute, clinical or skilled care setting is required.



15+ years of LPN experience and/or healthcare management experience will be considered in lieu of 5 years of RN nursing experience.



2 years of managerial experience in healthcare, preferably an acute, clinical or skilled care setting is required.



Clinical Case Management experience highly desired.




ADDITIONAL: Must demonstrate ability to communicate, perform and train the required skills necessary for providing routine nursing care both in a clinic or home setting. Must demonstrate competency and ability to use computer and working knowledge of computer based software as applicable. Must have excellent communication skills both verbal and written necessary to positively engage and interact with staff, physicians, participants and their care givers and facility visitors. Must be professional in appearance and manner, recognize own limits. Must demonstrate strong leadership, interpersonal and problem solving skills with the ability to foster a strong interdisciplinary teamwork environment. Responds and provides direction and guidance in a positive manner. Seek and provide help and guidance as appropriate. Serves as a role model for all staff members to include students, interns, and volunteers. Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects. Ability to move throughout facility and stand or sit for extended periods of time. Must be able to quickly respond in an emergency situation. Must be able to travel on as needed basis. This position may require climbing, kneeling, stooping, walking, pushing, pulling, lifting and restraining. Ability to analyze data and arrive at logical conclusions for operating and budgeting purposes. Comprehensive knowledge of general nursing theory and practice, to include basic disciplines related to nursing (such as biological, physical, social and medical sciences) with thorough knowledge of the underlying principles of care management, treatment and nursing procedures. Must maintain strong working knowledge of organization, policies and procedures of the facility. Requires higher level of comprehension, observation abilities, demonstrate memory for details, and exhibit independent decision making abilities. To also include knowledge of PACE regulatory policies, procedures and accreditation standards required. Frequent exposure to body fluids, needle puncture wounds and communicable diseases. Occasional exposure to toxic chemicals used for disinfection, hot water and specialty equipment that requires knowledge and training. Occasional exposure to chemotherapy agents.




HOURS: To work primarily 8:00am to 5:00pm Monday through Friday. Must be able to work varying shifts - evening shift, weekend shift and on-call as needed based on participant's care needs.
