Riverside PACE in Newport News, VA is currently seeking a full-time Social Worker MSW.
PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, Lifeline emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.
Job Summary: Works under the supervision of the Social Work Manager in accordance with the plan of care to provide quality therapeutic services for patients and their families. Assesses participants' psychosocial needs and interests and works with the interdisciplinary team to develop a care plan and coordinate appropriate services and care. Assists participants and families to adjust to changes in functioning and access appropriate resources in the community and facilitates communication between participants/families and the interdisciplinary team. Provides education and assistance with developing advance directives. Assists with crisis intervention for participants and caregivers. Facilitates caregiver support groups.
Education: Must have a minimum of a Master's degree in Social Work. Program must be accredited by the Counsel of Social Work Education.
Experience: Two years previous social work experience in a healthcare setting preferred. Must have at least one year previous experience working with the frail or elderly.
Licensure: CPR Certification and First Aid training required and must be maintained current throughout employment. Current, valid driver's license and acceptable driving record required upon hire and to be maintained during employment. Must also have reliable transportation to make visits to patient homes each day.
Hours: To work primarily 8:00a-4:30pm with additional hours as needed to meet the needs of the participants and facility.
