To work primarily Home Care patient assignments/cases through the PACE-MacTavish facility with rotation to the PACE-MacTavish Day Center as needed to meet patient care needs.
PACE (Program for All-Inclusive Care of the Elderly) is a community-based program which provides patients with all necessary health services while they stay in their homes. It covers many different types of services including a day center, personal care in the home, home based emergency response system, transportation to appointments, delivery of medical supplies, therapy, as well as inpatient services.
EDUCATION: High School graduate or GED equivalent required. Must also have proof of the following: A) Holds a certificate of completion from a State of VA approved nurse aide training program and can be registered by the Virginia Department of Health Professions as a certified nurse aide or B) Hold a certificate of completion from a professional nursing practical nursing or nurse aide training program approved by the Virginia Department of Education or the Virginia Community College System or the Virginia Department of Health Professions or C) Holds a certificate of completion from a home health aide training program approved by the Department of Health prior to July 1, 1984 and have been in continuous practice since that time.
LICENSURE: Licensed by the State of Virginia as a Certified Nursing Assistant (CNA) required. Proof of current, valid drivers license and acceptable driving record required upon hire and to be maintained during employment. BLS for Healthcare Providers Required.
EXPERIENCE: Previous the field of Personal Companion, Personal Care Aide, or Nurse Aide preferred. Must provide proof of minimum one (1) year experience working with the frail and/or elderly population in any capacity.
ADDITIONAL REQUIREMENTS: Ability to move about in the home and facility and stand or sit for extended periods of time. Ability to lift, push, stoop and pull in order to ambulate/transport patients assisted and unassisted. Must be able to exert up to 20-50 pounds of force occasionally and 10-25 pounds of force frequently and up to 10 pounds of force constantly.
HOURS: Ability to work primarily 3:00pm - 11:00pm, plus every other weekend. Available to work varying shifts (day shift, night shift and weekend shift) based on Day Center and participant's home care needs.
