Director of Home Healht & Home Care for Senior Options

The Director of Home Health and Home Care is responsible for the effective start up, transition and operations of Home health and Home Care throughout Senior Options.  This position will report to the Senior Director of Clinical Operations of Senior Options and team with co-workers to assure successful operations of home health.

 

RESPONSIBILITIES INCLUDE:


  • Serve as the subject matter expert in home health and home care.  
  • Remain current with regulatory, accreditation, certification, and reimbursement changes.
  • Assist supervisor with home health agency licensure, certifications and accreditations.
  • Manage home care and home health section of partner portal. 
    Update all policies, training materials and resources as required by regulation or as best practices evolve.
  • Understands home health diagnosis coding, OASIS, RAPs, Finals, Care Plans.
  • Provides training to home health administrators, clinical leaders, health information coordinators and others in home health processes, best practices, QAPI and new regulations.
  • Provides initial and follow up training to super users and home health leaders in use of electronic health record, submitting OASIS, submitting patient satisfaction surveys and other required procedures.
  • Leads integration of interdisciplinary teams for home health.  
  • Educate partners on best practices of clinical protocols, utilization management, and interdisciplinary communication.
  • Insures continuous quality improvement in publicly quality indicators such as the 5 star rating, patient re-hospitalization and patient satisfaction. 
  • Participates in monthly operations calls with partners.
    Facilitates quarterly home health team leaders’ meetings.
  • Remains abreast of health care issues that may impact Senior Options such as managed care, accountable care organizations, Medicaid and Dual Eligible managed care and Medicare Advantage private duty. 
  • Identifies clinical and operational issues needing improvement and connects the team to appropriate resources. 
  • Provides positive, responsive customer service as well as coaching, encouragement and support to WCAH and Senior Options partner staff.
  • May supervise additional clinical or operational Senior Options positions.
  • Joins senior leadership in account cultivation and pro forma development activities. 
  • Answers correspondence, email and voicemail appropriately and in a timely manner.
    When requested, presents speaking engagements and in-services
  • Frequent car or plane travel to Senior Options partner agencies.

Requirements

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

 

1.     Must be able to sit up to 8 hours a day.

2.     Must be able to stand up to 4 hours a day.

3.     Must be able to drive up to 8 hours a day.

4.     Travel up to 50% of time, including overnight travel to any state in the US where Senior Options has a partner. 

5.     Must be able to fly on commercial airlines within the United States.

Qualifications:

  • Bachelor’s Degree required, Master’s Degree preferred in health related field or management.
  • Registered Nurse in the State of Virginia strongly preferred.
  • Five years’ experience in health care, with a preference for home health and/or hospice experience.
  • Three years’ experience as a supervisor or administrator.
  • Demonstrated competency and prior experience using electronic health records.
  • Excellent computer skills.
  • Valid driver’s license and car in good working condition.
