The LPN, Home Health Nurse, assists the Registered Nurse Case Manager in carrying out the plan of care by providing skilled nursing care services. Perform routine visits and treatments as ordered. Document care provided.
Provide nursing care in the apartment setting to promote maximum health and independence for apartment residents. Incorporate appropriate resources and provide for continuity of care. Supervise Personal Care Assistants providing daily support services to apartment resident. Provide routine visits and treatments for clients/residents who have acute or chronic difficulties interfering with health, safety and independence. Make visits on campus and in the community. Document care provided.
DUTIES AND RESPONSIBILITIES:
- Provide services in accordance with organizational policies and procedures and regulations that define scope of practice.
- Respond to emergency calls from independent living residents. Perform emergency first aid and/or arrange for transport to Emergency Room.
- Visit residents upon request for services, including on return from hospital stay or level of care change to determine if personal care services are
needed. Makes referrals to other programs as needed.
- Prepare the clinical and progress notes documenting outcomes of interventions.
- Assist the RN and/or physician in performing specialized duties.
- Assist the RN in carrying out the plan of care.
- Prepare equipment and materials for treatment adhering to aseptic technique as required.
- Complete treatments, wound care, catheter care and medication administration as ordered.
- Assist the patient in learning appropriate self-care techniques.
- Assess patient/family response to care.
- Ensure communication of information to appropriate team members.
- Notify physician and Home Care Administrator of significant changes in client’s health status.
- Involve family members or other supportive persons as appropriate in assisting residents with their health care.
EDUCATION, TRAINING AND EXPERIENCE:
- One year of nursing experience, preferably in hospice, home health or a skilled nursing facility
- Unencumbered Virginia license as a Licensed Practical Nurse
- Current CPR certification (Basic Life Support)
REQUIREMENTS/QUALIFICATIONS
- Must possess good verbal and written communication skills
- Must be able to meet deadlines, work under limited supervision
- Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least 2 flights of stairs and be able to stand and/or
walk for a period of at least 4 hours.
- Must have a Valid state driver’s license
- Must provide Motor Vehicle driving record to Westminster-Canterbury and its insurance providers.
- Must provide Proof of motor vehicle insurance.
